| 6/24/26 | ![]() Episode 111: The Chaos Coordinator Badge | Alana was once gifted a "chaos coordinator" badge at her Clinic. She wore it. At the time, it felt like recognition. Looking back, she sees it differently.Because in General Practice, there is a culture of badges and mugs and shared trauma. The shit show supervisor. The captain of chaos. An identity built around how many fires you put out each day. That identity feels like belonging. But it can also become the trap that keeps Owners, Practice Managers, and teams stuck below the line. For Owners, this episode is a reminder that a Practice cannot scale if its leadership identity is built around rescuing, reacting, and absorbing every problem. In this episode, Todd sits down with Alana Flohr -- 13 years in General Practice, former Practice Manager of a large Tweed Coast Clinic -- to name the three behaviours that keep owners and PMs stuck in the drama, and the above-the-line alternatives that change everything. What you'll get from this episode:The three "below the line" roles -- victim, hero, and villain -- and how each shows up inside a real PracticeWhy the hero role is often the most seductive trap for Practice Managers and OwnersAlana's honest reflection on owning her own hero behaviour, including the chaos coordinator badge she once acceptedHow to coach your team through difficult conversations, even while you're still learning to have them yourselfThe "shut the door" system Alana used to protect one hour each afternoon for above-the-line thinking and improvement workTodd's recommended action for this week: share the episode with your PM, create a shared language, and agree to call it out when you see itResources mentioned:Dare to Lead by Brené Brown.....P.S. | 6/17/26 | ![]() Episode 110: The Paying Tenant Trap: What to Charge for Spare Rooms | Most Practice Owners with a spare room will let someone in before they've worked out what the room is actually worth. The logic is familiar: a paying tenant is better than an empty room. In the short term, it feels true. But below a certain rate, a tenant starts costing you more than the room generates -- in admin time, in blocked space, in the leverage you've quietly handed away. Todd and Sachin call this the keep-open cost, and most Owners don't know where theirs sits.In this episode, they walk through how to calculate it, who actually belongs in a spare room and in what order, and the agreement mistakes that lock Owners into the wrong arrangement for years. What you'll get from this episode:The hierarchy of what belongs in a spare room -- from a GP first down through allied health, and why the order mattersWhy the allied health tenant who treats your space like a business beats the sessional who comes twice a weekSachin's locked-in mistake: the five-year sublease that handed over front desk services and left no leverageWhy Todd watched a $700-800k allied health operation return almost nothing -- and what that taught him about keep-open costThe cosmetic injector scenario: why it sounds like easy income and why it almost always ends badlyThree mistakes to avoid: room block, below-cost rates, and the agreement that locks you out of your own best use.....Connect with Todd: LinkedInConnect with Sachin: LinkedIn.....P.S. After 25 years in banking, Scott decided it was time for a career change – one that involved more people and less paperwork. He joined The Surgery at Jerra in early 2023 after completing his Certificate III in Health Services, eager to gain hands-on experience in patient care. Originally planning to start a Bachelor of Nursing in 2024, Scott quickly realised he loved TSAJ too much to leave. He was promoted to Reception Manager in mid-2024 before stepping up the ladder in 2025.A father of five, Scott is no stranger to multitasking, organisation, and a little bit of chaos—all skills that serve him well in keeping the practice running smoothly. When he’s not supporting the TSAJ team, he’s busy being a dedicated groupie, cheering on his partner as he rocks out on the drums in a pipe band.Scott also loves gardening and discovering new coffee spots, so much so that he and his partner are setting up their own coffee van to run on weekends. Because if you can’t find the perfect brew, why not serve it yourself?CONNECT WITH SCOTT WEAIREWebsite: https://tsaj.com.au/team/ABOUT KYLIE YATESKylie YatesTSAJ Co-ownerAs the owner and business manager of The Surgery at Jerra, Kylie is responsible for the bigger picture—strategic planning, financial operations, and making sure the practice runs like a well-oiled machine (or at least as close as possible!). With a strong focus on care, compassion, teamwork, and trust, she’s passionate about building a thriving workplace culture where both staff and patients feel valued. Before stepping into the world of general practice, Kylie had an extensive career in broadcast media and communications, working as a news journalist, presenter, newsroom chief of staff, and producer for multiple television networks. She’s also worked in communications for the Australian Medical Association and as a media adviser to a senior Federal Cabinet Minister across three portfolios—so she’s no stranger to high-pressure environments! At home, Kylie and Dr David Yates have three teenagers together, and she’s a ‘stepmonster’ to two very big boys. A long-time Brumbies fan, she’s even spent years coaching junior rugby for the Queanbeyan Whites (why just watch when you can get involved?).When she’s not overseeing the practice, wrangling teenagers, or watching rugby, Kylie enjoys traveling, crosswords, and the idea of going bike riding—though finding the time and energy is another story! She’s recently taken up CrossFit, but she’s the first to admit she’s more ‘CrossUnfit’ than a ninja warrior. And while some people swear by coffee, Kylie is firmly in the tea camp—preferably enjoyed in peace and quiet, though she knows that’s wishful thinking!CONNECT WITH KYLIE YATESWebsite: https://tsaj.com.au.....P.S. | 1/14/26 | ![]() Episode 88: Scaling in an MMM5 Town: The Real Story Behind Growing From 2.3 to 8.5 FTE GPs | How do you scale a clinic without burning out or breaking what made it special? In this episode, Dr Todd Cameron sits down with rural GP owner Dr Ben Nally for a deeply honest, practical conversation about what it really takes to scale a General Practice, especially when you’re doing it in a small, regional community with limited workforce, tight margins, and very public accountability. You’ll enjoy a grounded walkthrough of what happens when a clinic starts in the red, runs on goodwill instead of systems, and slowly — deliberately — transforms into a thriving, sustainable business that serves both patients and the people running it.Ben shares the uncomfortable early lessons, the small strategic wins that created momentum, and the leadership mindset shifts that allowed the clinic to grow from 2–3 FTE GPs to a team of 14+, without burning out the owners or losing the soul of the practice. This conversation speaks directly to owners who feel the weight of responsibility — to patients, staff, and community — but don’t want to sacrifice their health, family, or sanity to keep the doors open. It reinforces a core truth: sustainable growth doesn’t come from doing more yourself; it comes from better systems, better people, and clearer leadership.Stop reacting, start leading intentionally, and build a practice that actually supports your life, not consumes it.In this episode of the Scale My Clinic Podcast, you’ll learn:00:57 - The realities of running a practice in a rural community03:30 - GP Ownership as community leadership, not just business10:31 - Buying into a legacy practice (and knowing what to change and what not to break)13:16 - Mechanics of turning a struggling clinic around and how small wins create momentum16:44 - How intentional investment in people, leadership, and the physical clinic environment unlocks growth from 2.3 FTE GPs to 8.5+ FTEABOUT BEN NALLYDr Ben Nally, JCCA, MBBS, FRACGP is a Practice Owner, General Practitioner, Post Graduate Diploma in AnaestheticsWHERE TO FIND BEN NALLYWebsite: https://cgpmansfield.com.au/doctors-and-staff/#ben Website: https://cgpmansfield.com.au/ Missed the previous Episode?Listen to Episode 87 here: Scaling Clinics Without Losing Lifestyle: How Mary & Sean Did It.....P.S.
